I was investigating the risks of sheath cleaning and came across this article which recommends leaving the sheath entirely alone unless a specific problem is diagnosed. The source is the AAEP, which I would expect to be authoritative.

Hello Mark,
I think people over wash the sheath. However washing the sheath once a year (as recommended in our article) gives you a chance to explore the sheath and penis for abnormalities and it is important to check for the bean periodically. I have found cancers on these exams and I have seen horses with huge beans that the owner reported seemed to be uncomfortable when urinating. For the rest of the year a good rinse with just plain water after a ride to rinse out the sweat and perhaps any excess oils will do just fine. I do this my gently running the running hose up the sheath. If I don't do this one of my geldings get very gooey and odorous during the heavy riding season.
