Discussion on Recommendations for hematoma treatment after removing drain

I'm treating a horse who had a hematoma/seroma (on his hip) drained 12 days ago. The drains came out after 7 days, and the incisions are trying to close up, but there is still quite a lot of serum production, and I'm wondering what other recommendations you would have for treating this. The vet was having me flush it with saline; how long should that be kept up? What is the best way to keep the incisions open and draining, and also keep them clean?

Lisa, I would recommend you use the hose with a foreful stream. This procedure is explained in » Equine Diseases » Skin Diseases » Long Term Deep Wound Care. It is imperative that a wound have ventral drainage, that is drainage out the bottom on the wound. DrO

Thanks, DrO. The bottom incision has healed closed and I can't reopen it now without a scalpel, but the incision above it I have been able to keep open. The wound isn't draining on its own, but I have been draining it twice daily with a 60cc syringe. Do you think I should have another drain put in? How long do these things usually take to heal? One more thing - after I've drained the wound, the skin lays flat and tight against the muscle underneath, but the whole area is raised about 1/2" or so, there's a definite edge to it. He doesn't seem tender in that area, the only place he's touchy is on the incision itself. Thanks again!

Yes the bottom drain needs to open and draining. Time for healing dopends on the severity of the intial insult, and complications like this. I have left drains in for up to 2 weeks. DrO
