Posted on Friday, May 18, 2007 - 7:09 am: Hi Dr. O,I'm doing equine rehabilitation (I"m a licensed human PT) and I got a vet referral for a horse with a gluteal mm tear. I treated it with laser therapy with great results. The horse's history is a percheron/tb who has in the past had severe tying up. He has been on a low carb diet and doing well. This March/April they went to SC for a 2 weeks riding vacation. He was very fit when he left and got even fitter while down there. He did not do well with the trailering and needed to be tubed with fluids when he arrived. He also was a nervous reck for several days. At the end of their stay he tore the muscle when descending a very steep embankment. He also looked a bit dehydrated when he returned to NH. Once back in NH, I saw him. He also had a bubble and slight scarring on the upper part of his semi membrenosis. He was on turnout in a sm paddock (his normal turn out) Over the last 4 weeks the exercise has gone from walks with no hills to trotting on the straight and adding in ring work at the walk only. she had started going on hacks with others and also cantering. He looked great, but there is ever so slight separation still in the mm. Then he torn another mm on the other side-It appears to be distal part of the gluteal superficialis. This is large swollen and not painful. The vet was out and there is no history to give a reason for this new injury-a healthy fit horse shouldn't just tear mm? they took blood to see if there is something like Se or E issues. The horse has been supplemented with these for at least 7 years since his originally tying up issues. His pelvis appears level. Can the dehydration form his trip cause long lasting issues that would make him more susceptible to the mm damage? Thanks, Kim |

Posted on Monday, May 21, 2007 - 6:41 am: Hello Kim,Hmmm predisposed to future episodes of myopathy caused by a past episode of dehydration...unless there is some sort of permanent pathology that I cannot think of, it seems unlikely. How is the muscle tear being diagnosed? The lack of pain strikes me as a odd presentation for a acutely torn muscle. DrO |

Posted on Monday, May 21, 2007 - 7:05 am: The vet is doing blood work as she is very puzzled. The fibers are broken across the mm belly unlike the previous tear. He is in a paddock alone, but I think perhaps his neighbor who he plays with across the fence-6 " wide flexible fence material (not real boards) may have kicked him. It is strange that he isn't sore.Thanks, Kim |
