Discussion on Nosebleed after exercise on horse with IAD

Hi Dr. O, My 11 yr old Hanoverian was diagnosed with Inflammatory Airway Disease most likely due to allergies, according to my vet. He has been on a nebulizer for about 15 days. The solution is 20cc of cromolyn sodium, salmeterol and budesonide. The first 10 days he got nebulized twice a day and he has been on once a day for the past 6 days. This is the first week that I have worked him on consecutive days, the last few weeks being light, intermittent work. I am still keeping his work light, but did trot him out a little longer than normal yesterday. When we got back to the barn, he had a slight nosebleed that lasted about a half hour and then stopped. Do you think the nosebleed is from the medication he is inhaling or from the exercise itself. I don't know whether to continue his treatments (we have about 7 days left) or stop riding him or both. My vet is away until Monday and I don't want to overlook something that could potentially be very serious. Any thoughts?

Hello Dana, While we don't know the source / cause of the nosebleed, I think discontinuing riding, at least till he can be examined, is sensible. IAD can cause significant disease and I do not see a clear reason to stop them but this is not my judgement to make. Dr. O, Can those medications thin out his nasal passages, causing them to bleed? Have your heard any cases of the nebulizer causing nosebleeds? I did read the articles on nosebleeds, but I didn't see any information on nebulizers and their effects on the nasal passages.

Hello Dana, I do not have personal experience with these medications and there is not a lot of information about this combination. Looking at the individual components they can be irritating in some individuals and the steroid component, budesonide, might overtime cause a thinning of the tissues. While nosebleeds are listed side effect it is used to treat conditions that cause nose bleeds also so I wonder if it is causal. DrO
