Hi Dr. O, My 9 year old Friesian x TB mare came in from the field a little over two weeks ago with swelling of the tendon sheath in her rt. hind leg, with noticeable swelling around the fetlock (like huge windpuffs). She was not lame and had only a little heat in the leg. The vet advised a regimen of bute, cold therapy, stall rest with short peroids of hand walking, standing wraps, DMSO alternating with clay poultice. After two weeks there was no heat, but the swelling was only slightly improved. She still was not lame. So I had an equine specialist out. He did flexion tests and she was not lame at all, did not react in the slightest to him flexing her. He did an ultrasound and the only problems viewable were the fluid and the annular ligament was enlarged. He recommended continuing the same regimen but we ordered Surpass to replace oral Bute. I just switched her today to the Surpass. He said that he could inject with cortizone but that he would prefer to give the stall rest a few more weeks to see if it resolves. Everything I have read about other horses with a problem with their annular ligament have been lame to some extent. I am kind of puzzled about this and wondering if there is something more I should do or be looking for... Any thoughts are appreciated. Brenda

Hello Brenda, The annular ligament is a "little" enlarged but by how much and in what ways is it enlarged? This may not be a significant finding Brenda. It can be a little enlarged but not cause problems with surrounding structures or be painful itself. DrO

Dr. O, I guess I should have said she was a little swollen. The annular is a lot enlarged. 15-18mm. The tendon sheath is swollen. There is no evidence of lesions of the tendon.

I think the advice given so far is sensible for a flexor synovitis (swollen sheath) without lameness. Concerning the possibility of a annular ligament desmitis the article has specific recommendations but a longer rest period is likely needed. DrO
