Discussion on Eye ulcer won't take stain

We have a mare with an ulcer way up under the lid in the corner of her eye that will not take stain. She is on anti-bacterials, anti-fungals, atropine, banamine, multiple times a day. Symptoms are squinting, tearing. The size is going down. We reduced the frequency of the meds a few days ago after the vet rechecked it and confirmed it was getting smaller. Yesterday morning her eye was 1/2 closed again, but after medicating it looks ok again. We are wondering about getting a lavage system put in. Also, they haven't done a culture or sensitivity. Wonder if that should be a next step. It remains non-staining. I am just wondering if the drugs can access the infection/ ulcer properly if it is essentially healed over? Also, it should be noted that the mare is in foal and due in about 10 days. Milk is not white yet. Thanks!

Hello Judy, I assume we are talking about fluorescein stain? The stain is sensitive to the fibrous portions of the eye and does not stain the thin epithelial layer that covers it. If we assume your assessment is correct, you have a corneal ulcer present, then it suggests that the epithelial layer has covered it over before the fibrous portion healed completely. In order for the fibrous portion to heal it usually requires blood vessels to invade the cornea. If it continues to improve it sounds like you are on the right track. If you are uncertain you should consider a referral to a ophthalmologist where culture, histopath, and possibly just freshening the ulcer to stimulate neovascularization may all help define and treat the problem.
