Discussion on Sinus Bone Fracture

I have a mare that had a puncture wound just to the left of her eye that was sutured but after three days the stitches came out. The wound seemed to be healing fine until I noticed more swelling and some milky discharge about a month later. The vet came back and opened up the wound tonight to make sure there was nothing still in it. When she was flushing the wound with betadine it started dripping out of her nose instead of coming back out of the wound. Her conclusion is that the sinus bone is fractured. She thought she could feel the bones overlapping but no loose pieces. She said that the only thing to do is to leave the wound open to drain and give her an antibiotic. She gave her penicillin and is going to call me tomorrow to tell me what antibiotic works best for a bone infection. Does this seem like a reasonable approach?

Hello Scott, If she is confident that there are no missing fragments nor large fractures yes this sounds reasonable if you add daily local wound care. DrO

Just an update. The puncture wound seems to be healing although there is still some drainage. Yesterday morning I noticed a large knot in the middle of her forehead (about three inches above and to the left of the puncture wound). By evening it had ruptured and was draining. The vet opened the abscess large enough to feel inside and could feel a small displaced ridge on the bone. She has never found any fragments. That would make the break at least five inches long. She gave another injection of some type of long-acting antibiotic and plans to give another on Saturday. She thought that it is just an infection caused by the fracture and there isn't much more to do. I clean the wounds twice a day with betadine and put on an antibiotic cream.
