I read the info on PO but still do not understand what exactly constitutes PO and why it causes soreness. My 11 yr old QH gelding was just diagnosed with PO in both front feet..and according to what I have read PO is only unilateral..? X rays and nerve blocks were used to achieve the diagnosis on my horse. The right front is worse than left.

Your confusion is understandable. PO is not a diagnosis: it is a description of a disease process: inflammation in the bone. These leads to the question: what is causing the inflammation? Documented diagnosis that can cause PO are infection, trauma, and cancer. These are problems that do not tend to be bilateral problems, though it is not impossible.

One problem with diagnosing inflammation of the coffin bone from radiographs is that the coffin bone in sound horses can be very irregular: is that irregularness inflammation or a variation of normal.

What would be your recommendation for shoeing for a horse "diagnosed" with PO? My horse doesn't have cancer (don't think anyway) I don't know of any specific trauma he has sustained..his lameness has been more of a gradual worsening of "tender feet" culminating with him now being noticeable off on hard ground especially when trotted in a circle and worse in the right. The xrays showed irregular shaped coffin bones and not very good density with "holes" in toe area..worse in right. Thank you
