My 13 yo gelding has findings on exam highly suspicious for ulcers. He has been on bute intermittently, and devil's claw almost daily for the past 3 years for chronic laminitis, stone bruises, foot soreness, etc. I am no longer using bute or devil's claw. My vet recommended 1/2 tube of Ulcerguard x 14 days, then return to U-Guard and Probios. I'm thinking that the treatment and maintenance course may need to be much longer than 14 days.

I have been in contact with a pharmacist that can provide compounded omeprazole at a fraction of the cost of Ulcerguard and Gastroguard. I realize most studies say compounded omeprazole is inferior, but I noted in one of the articles that if the vehicle pH is >8, it is efficacious. If the pharmacist can mix compounded omeprazole in a vehicle with pH >8, is it worth trying on my horse? The cost savings is substantial.

Hello Denise,
They do believe that the primary problem with compounded omeprazole was one of too low a ph. If you can monitor the progress of the ulcers effectively you could consider trying the compounded formulation and then switch if you don't see results. The problem is the word "suspicion" you use above. It makes me think it may be hard to monitor results. If you see little improvement you won't know if it is the diagnosis or treatment.

We can moniter cinchiness and grumpy attitude when his abdomen is palpated. It is quite significant. He has lots of the additional anecdotal symptoms of ulcers that I've been reading about. Will keep an eye on all of it, and the vet will be following up.

If it gets too confusing, we will get endoscopy, but I like the simultaneous diagnostic and therapeutic approach for now. Clinically he is a setup for ulcers, and omeprazole is worth a try. Just trying give him an adequate trial of medication, but also be sensible about spending money.
