Discussion on Bute and Prednisolone

Good morning, Dr. O. Other than an increased risk of ulcers are there any contraindications against using bute and prednisolone during the same course of treatment? Also, I read somewhere that bute and previcox work on different levels of pain receptors and it may be helpful to give both for a few days if previcox alone is not working. The article I read did state this wasn't recommended long term. What are your thoughts on this? Thanks, in advance!

Hello Sara, Ulcers are the primary concern when steroids and NSAIDs are used together and a important contraindication. Both phenylbutazone and firoxcib work by inhibiting the cycloxygenase pathways which are both the source of pain relief and toxicity. I would not add bute to firoxcib, the bute is likely to be more toxic. If you are willing to accept the increased risk of ulcers, I believe it would be safer and just as effective to increase the firoxcib. DrO

Just MY experience, that may not apply to you, Sara: I've had a mare on bute since 1986 to 2008 when she was switched to Previcox. She has also been on pergolide since 2005. We have agreed since back in the eighties, that once these drugs harmed her, or failed to arrest her pain, we would let this mare go....she's 33 now...

Thanks for the input Lee. This situation is a little different. I'm looking for the most aggressive treatment for inflammation I can find. I'll treat to prevent against ulcers but they are the least of my concerns at this point. If it works, we shouldn't have to have long term use of either of the drugs. If it doesn't...well, we won't talk about if it doesn't because it will!

Hey, Sara: I just wanted to point out that not EVERY horse will have a guaranty of ulcers with bute. I also have an appendix mare that can get an instant ulcer attack with bute, unless treated with gastroguard simultaneously. Unfortunately, treatment necessity arises for her when she indulges her "crazies" and irritates the navicular in her LF. We just do the cycling of gastro and bute to minimize the inflamation and keep the threat of ulcers to a minimum. The straight quarter mare has never needed nor received gastroguard or ulcerguard-in twenty-five years or more...go figure.

Not every horse? Almost 30 years of bute and never had a case of clinically evident ulcers with mostly judicious and the occasional aggressive use in horses. I am careful but there is not doubt that the general impression that bute commonly causes symptoms of ulcers is just plain wrong. I have prescribed many thousands of doses. DrO
